At River Valley Dental we pride ourselves on giving you convenient options, so you have a few choices when it comes to filling in your paperwork. You are welcome to arrive 15 minutes early for your appointment, download the forms below, or visit our patient portal.

In addition to the above options, we offer in-house financing plans to make payments more affordable to our patients.
We are often times asked if a patient can set up a payment plan. Although we prefer to collect payment in full prior to the completion of a service, we do understand that this may not always be possible. We try to make dental costs affordable for all patients. We cannot change our fees, as we have to file them to keep in compliance with programs we participate with, but we can try to help you find a way to make them work with your budget.
In Office Financing
In-office payment plans are available on a limited basis. If payment in full is not feasible at the time a patient completes treatment they will need to discuss any financing with our Office Manager, Jessica, prior to the completion of the treatment. Balances carried from month to month may incur a finance charge and/or billing charge.
We accept CareCredit. Care Credit is an out of the office, revolving line of credit that requires a credit application. Upon approval, our office can offer the 12 months “no interest” ¹ plan or 24-60 month “low interest” ² plan dependent on the amount financed. Care Credit is accepted at other offices in the area including some oral surgeons, chiropractors, eye care clinics, or veterinary clinics.
¹If paid within the promotional period. Otherwise, interest assessed from purchase date. Minimum monthly payment required.

Whether you have insurance or not, it should never stop you from caring for your smile.
If you have dental insurance, we are happy to submit your visit to your insurance plan on your behalf. We are in-network with Delta Dental Premier, Delta Dental PPO, and Delta Dental Group 216.

No Insurance? No Problem.
Making quality dental care accessible to all!
For our patients without insurance who prefer to pay over time and receive a discount on treatment, we have a patient membership program. With this program, you pay a lifetime activation fee that includes your exam, cleaning, and necessary x-rays. Then for a small monthly fee, your preventative care is included along with the opportunity for discounted treatment.
You save between $111 and $128 annually by becoming a member versus just paying for these services out of pocket as you go.
Your First Experience at River Valley Dental
Take a sneak peek at what happens during a typical visit at our office.
Our upbeat front desk team welcome you by name to our office and help you with any outstanding paperwork. We endeavor to answer any questions you have and escort you to your treatment room.
Once you are comfortable in the treatment room, Dr. Pooley will spend some time getting to know you and identifying your chief concern. He then carries out a full-mouth exam using x-rays and discusses his findings.
When you are comfortable and satisfied with a treatment, we produce a plan for you and discuss your financial options. Our front desk team will help you schedule an appointment and go through any questions you may have.
